When you schedule an appointment, you are agreeing to these terms and conditions.
You agree that you are at least 18 years of age or have a guardian over 18. For clients under the age of 18, guardian MUST sign all intake informs and be present at ALL appointments.
In order to ensure that you obtain your desired appointments, we recommend scheduling your appointment as far in advance as possible to ensure availability.
Details of our refund and cancellation policy are provided below.
We are unable to process any refunds or reimburse any payment transaction on any service, packages or service series that are purchased. There are also NO Refunds are issued on completed services. While we make every effort to make our customers as happy as possible, we are a small business and unfortunately cannot offer refunds on any used or unused appointments/sessions for individual services, service packages or service series. A complimentary service at the FULL DISCRETION of spa manager is the sole remedy.
I understand that a 50% deposit is required to secure my appointment and that deposit is applied to the total cost of the service and is NON-REFUNDABLE.
I understand that ANY unused treatments will expire after 12 months from purchase date. Monetary refunds are NOT issued for unused services. In the event that a client cannot continue with treatments, all unused services are NON REFUNDABLE
Rescheduling/No Show Policies and Fees
Your treatments are reserved especially for you. We value your business and ask that you respect the spa’s scheduling policies. Should you need to reschedule or make changes to your appointment, you can do so at least 24 hours in advance via email, phone call or through your online booking dashboard. Appointments can be rescheduled online on your Booking Dashboard Page. In the event that client experiences any technical difficulties with their booking dashboard, client can send an email or call us to cancel or reschedule. Technical difficulties with the website or electronic communication does NOT exempt client from the required notice and ALL cancellation terms still applies.
ANY cancellations/reschedules made less than 24 hours of your scheduled appointment is non refundable and customer is responsible for 100% of the treatment cost. I understand that treatments rooms, equipment and staff are solely assigned to their appointment and that any no-shows, late cancellation (without a minimum of 24 hours notice), will be charged 100% of the full cost of the treatment and is nonrefundable.
Rescheduling/No Show Policy for Group and Custom Appointment(s) – For custom packages, a minimum of 7 days prior to appointment time is required to make any modifications to the appointment. Cancellation made within the 7 days and at least 48 hours before the scheduled appointment will be charged at 50% of the cost of service to reschedule. After 48 hours customer(s) is responsible for 100% of the service and cost is non-refundable.
CHIOMA SPA AND OUR STAFF PRIDE OURSELVES ON ENSURING LOW COST TREATMENTS AND THAT ALL OF OUR CLIENTS ARE SEEN ON TIME, WITHOUT ANY UNNECESSARY WAIT TIMES.
TO ACCOMPLISH THIS, WE RELY UPON OUR CLIENTS COMMITMENT TO KEEPING THE APPOINTMENTS THEY HAVE SCHEDULED FOR THEIR CONVENIENCE. TO ENSURE THAT YOU ARE SEEN PROMPTLY AT YOUR DESIRED TIME, WE STRICTLY AVOID ANY OVERBOOKING. YOUR TECHNICIAN IS ASSIGNED TO YOU AT THE APPOINTMENT TIME YOU HAVE REQUESTED. CHIOMA SPA’s PRICING IS CAREFULLY CALCULATED UPON OUR CLIENTs ABILITY TO KEEP THE APPOINTMENTS THEY HAVE COMMITTED TO. THEREFORE, PLEASE NOTIFY IN ADVANCE IF YOU CANNOT KEEP YOUR APPOINTMENT SO WE HAVE THE OPPORTUNITY TO SCHEDULE ANOTHER CLIENT WITHIN YOUR APPOINTMENT TIME. When you do not notify us in time, we not only lose your business, but also the potential business of other clients who could have scheduled an appointment for the same time
AFTER THREE CONSECUTIVE NO-SHOWS TO YOUR APPOINTMENT, ALL REMAINING SCHEDULED APPOINTMENTS WILL BE CANCELLED AND ANY NEW APPOINTMENTS WILL ON A CALL IN BASIS ONLY.
Inclement Weather Policy
In the event of extreme weather advisory, we will work with customers to reschedule to a later date. Please be advised ANY purchased service is NON-REFUNDABLE.
Other Unforeseen Incident Policy
FORCE MAJEURE. CHIOMA will not be liable for any delay or failure to perform its obligation under this Agreement if prevented from doing so by acts of God, acts of war or civil disturbance, pandemics, governmental restraints, utility or communications failures, or any other causes that the affected party could not, with reasonable care, control or prevent. If such an event occurs, ANY purchased services, packages, gift treatments or gift cards are NON-REFUNDABLE, and the affected party will use reasonable efforts to promptly resume its obligations under this Agreement upon cessation of the Force Majeure event. CHIOMA will work with the affect party to reschedule appointment affected by the force majeure event to the best of its abilities.
All appointments have been designed to allow appropriate time for the completion of each service. You are granted a maximum 10 minute grace period at the FULL discretion of your technician and based on capacity. Please be aware that late arrivals will not be afforded extension of scheduled treatments. You will still be responsible for payment for 100% of the service cost and payment will be non-refundable.
We recommend that you arrive at least 15 minutes prior to your first scheduled appointment. This will allow ample time to fill out a client profile necessary to tailor our services to your personal needs.
Please advise us at time of booking of any health conditions, allergies, injuries, recent surgeries, pregnancy or special needs which may affect your services. In the event of any changes to your health condition, please notify us immediately. If service or packaged purchased cannot be rendered due to health condition, we can exchange them for credit towards another service or package of equal value.
Prices and Services
Prices and services are subject to change without notice. We will do our best to keep our clients up to date on any changes that may occur but encourage all clients to inquire about pricing and available services when booking your appointment.
Gratuity is not included in the price of service or packages, customary tipping runs 15% to 20% but is, of course, at your discretion. You may tip the technician upon check out at the front desk.
We ask that you please arrive 10-15 minutes prior to your appointment time. We will do our best to accommodate late arrivals. However, the length of service may be adjusted so as to not interrupt the scheduled appointments of other guests. Full price of scheduled services will apply, so please plan accordingly.
Please leave all jewelry and valuable items at home or keep them with you at all times. CHIOMA is not responsible for lost or misplaced items.
Our technicians are here solely to help improve your wellness. If a technician ever feels that a client is behaving inappropriately, they are authorized to stop the treatment immediately and direct you to the front desk.
CHIOMA Gift Cards and E-Gift Cards should be treated as cash. All sales are final; Gift Cards and E-Gift Cards cannot be exchanged or refunded for any reason. To provide maximum protection to your credit card account, and to allow time for your order to process, our e-Gift Cards contain value that may be used to purchase services, however they cannot be used to purchase other gift cards or e-gift cards.
Redeeming Gift Cards
Gift cards are ONLY redeemable at CHIOMA for the amount or service specified on the gift cards.
Gift cards CANNOT BE not redeemable for cash and cannot be returned for a cash refund. Any unused balance of the gift card will remain until redeemed.
If a purchase exceeds the amount listed on the gift card, the redeemer is responsible for paying the difference at time of purchase. For security purposes, a recipient will be required to show valid ID before he or she can redeem the gift card.
Cancellation and Expiration of Gift Cards
CHIOMA reserves the right to immediately invalidate gift certificates, without notice, should it receive a credit card chargeback, returned check or other form of repudiated payment made towards the gift card.
CHIOMA reserves the right to dishonor the gift card, if it reasonably believes it was not validly purchased, has already been redeemed, or the redeemer stole or is otherwise not the intended beneficiary of the purchaser.
Gift Card cannot be used for gratuities or tips.
Gift Cards Purchases Expire 5 years from Purchase Date
Miscellaneous Terms and Conditions
CHOIMA reserves the right to change these Terms and Conditions from time to time in its sole discretion.
CHIOMA is not responsible for emailed gift cards orders that do not reach their intended recipient for reasons beyond its control. Bounced email is not monitored. Receipt of email cannot be verified.
CHIOMA, ITS AGENTS AND AFFILIATES, MAKE NO WARRANTIES, EXPRESS OR IMPLIED, WITH RESPECT TO GIFT CARDS, INCLUDING WITHOUT LIMITATION, ANY EXPRESS OR IMPLIED WARRANTY OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE
Gift cards, and these Terms and Conditions, are subject to and governed by the laws of Texas, without reference to its principles of conflicts of law.If any provision of these Terms and Conditions should, for any reason, be held invalid, prohibited by law or unenforceable in any respect, such term will not apply. However, the remainder of these Terms and Conditions shall be enforced to the full extent permitted by law.
Risk of loss will pass to the purchaser of the gift card upon the electronic transmission of such gift card to the recipient or purchaser.
I understand that with any treatment certain risks are involved and that any complications or side effects from known or unknown causes could occur which may include and are not limited to reddening, pigment changes, altered sensations, discomfort, swelling, scarring, bruising and permanent changes to body symmetry. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, AND HOLD HARMLESS, RELEASE, INDEMNIFY AND DEFEND THE TECHNICIAN, CHIOMA BODYCON, CHIOMA Co, ITS SUBSIDIARIES AND AFFILIATES, THEIR RESPECTIVE OFFICERS, DIRECTORS, AND EMPLOYEES FROM ANY LIABILITY, CLAIMS, DEMANDS, ACTIONS AND CAUSES OF ACTION WHATSOEVER WHICH MAY BE ASSOCIATED WITH AND/OR RESULT FROM MY INVOLVEMENT IN SUCH AN ACTIVITY AND/OR ARISING OUT OF OR RELATING TO ANY SERVICES AND/OR SPA TREATMENT OBTAINED BY ME AT CHIOMA BODYCON EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation.
I understand and voluntarily accept the risks associated with the use of CHIOMA’S location’s facilities including but not limited to equipment, treatments, treatment beds, chairs, products and accessories. I agree CHIOMA will NOT be liable for any injury, including, without limitation, personal, bodily or mental injury, economic loss or damage to me resulting from negligence, other acts i, anyone acting on CHIOMA’s behalf, or anyone using the services of the facilities of CHIOMA, to the fullest extent permitted by law. This agreement together with CHIOMA rules and regulations, constitute the entire agreement between you and us and cannot be amended, except in writing by both parties. Myself and/or any of my heirs, executors, representatives, or assignees hereby release CHIOMA from ALL claims or liabilities for death, personal injury or property loss or damages of any kind sustained while on the premises and/or from any advice or services provided by an employee, independent contractor or any representative of CHIOMA. I agree that this application and waiver is in effect for ALL Sessions or any other services, and will NOT expire.
